WHAT WE NEED FROM YOU
Damon Tax Service strives to make tax season as easy as possible for our clients. In order to assist you with your tax preparation needs as efficiently and as cost effectively as possible, please be sure to:
1) Print or download the CLIENT INFORMATION SHEET, fill out all lines completely, sign, date and return to Damon Tax Service via Email to your preparer, fax, drop off to the office or bring with you to your scheduled appointment. If something doesn't pertain to you, please write N/A next to it.
If you are an existing client and drop off your tax information to us, please include this completed form with your documents.
Whether you are an existing client or new with us, we must ask that you fill in all information on the client information sheet each tax season. This includes all dependent and personal information, direct deposit info etc. MISSING OR INCOMPLETE INFORMATION SHEETS WILL DELAY THE PROCESSING OF YOUR TAX RETURN.
2) Provide us with all tax documents such as W-2's 1099's, interest statements, property taxes docs, daycare provider information, etc. You can reference the TAX DOCUMENTS CHECKLIST to prepare your documents and info for tax prep.
If you collect unemployment during tax year, you need to go online to get your 1099G.
If you have a business or rentals, be sure to categorize and total the income and expenses.
If you use your auto for business purposes provide us your totaled mileage log with complete information.
Bring your organized receipts with you in the case we must clarify an expense
NEW CLIENTS AND CLIENTS WITH MISSING/INCOMPLETE INFORMATION MAY NEED A SECOND APPOINTMENT TO COMPLETE TAXES.
